Independent Care Waiver Program (ICWP) Overview:
The ICWP offers services to a limited number of adults who apply Eligibility for ICWP is based on either a nursing facility or hospital level of care for adults with severe physical disabilities or traumatic brain injury (TBI).
The Independent Care Waiver Program (ICWP) offers services that help a limited number of adult Medicaid members, between the ages of 21 and 64 with physical disabilities, to live in their own homes or in the community instead of a hospital or nursing home. ICWP services are also available for persons with traumatic brain injuries (TBI).
Hope Private-In Home Care, LLC (Hope Private Care) is certified by the Georgia Department of Community Health, to provide these In-Home Care services at this enhanced care level, to younger adults 21 to 64 years old, throughout Metro Atlanta and the surrounding Counties (see our listing of Service Areas covered).
Our Nurses and Caregivers work with the State appointed Case Managers, to provide all approved in-home care services, Skilled Nursing, Wound Care, etc., once you or your loved one is admitted into the program. Examples of the services our Nurses have provided include, but are not limited to:
• Foley Catheter Care and Replacement
• Tube Feeding
• Ventilator Care
• Wound Vac / Wound Care
• Tracheostomy Care
• Suctioning and General Hygiene Care
Individuals who are considering nursing home or other institutional care may be eligible for home- and community-based services as an alternative through Georgia’s Medicaid waiver program.
Applicants are offered the choice between community-based services or institutional care as long as the community services do not cost more than the institutional care.
Additional available State Approved services could include personal support, home health services, specialized medical equipment and supplies, emergency response systems, and home modifications.
